THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
YOUR RIGHTS
You have the right to:
Get a copy of your medical record
You can ask to see or get an electronic or paper copy of your medical record and other health information we have about you related to your outpatient physical and occupational therapy care. We will provide a copy or summary, usually within 30 days of your request. We may charge a reasonable, cost-based fee.
Correct your medical record
You can ask us to correct health information about you that you think is incorrect or incomplete. We may deny your request, but we will explain why in writing within 60 days.
Request confidential communications
You can ask us to contact you in a specific way (for example, by phone only or at a specific address).
Ask us to limit what we use or share
You can ask us not to use or share certain health information for treatment, payment, or healthcare operations. We are not required to agree to all requests.
If you pay for a service or health care item out-of-pocket in full, you can ask us not to share that information with your health insurer for purposes of payment or our operations. We will comply unless a law requires us to share the information.
Get a list of disclosures
You can ask for a list of certain times we have shared your health information for six years prior to the date of your request, who we shared it with, and why.
Get a copy of this notice
You can ask for a paper copy of this Notice at any time, even if you have agreed to receive it electronically.
Choose someone to act for you
If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information.
File a complaint
You can complain if you feel your rights have been violated.

For certain health information, you can tell us your preferences about what we share. If you are unable to tell us your preference, we may share information if we believe it is in your best interest.

We use your health information and share it with other healthcare professionals involved in your outpatient physical and occupational therapy care.
Payment
We use and share your health information to bill and receive payment for outpatient physical and occupational therapy services, including Medicare, Medicaid, the VA, and private insurance plans.
Healthcare Operations
We use and share your health information to operate our practice, improve care quality, and manage outpatient physical and occupational therapy services.
OTHER USES AND DISCLOSURES PERMITTED OR REQUIRED BY LAW
We may also use or share your information:
For public health and safety activities
For health oversight activities
For judicial or administrative proceedings
For law enforcement purposes
For workers’ compensation claims
To prevent or reduce a serious threat to health or safety
We will never sell your health information or share it for marketing purposes without your written permission.

Maintain the privacy and security of your protected health information
Notify you promptly if a breach occurs that may have compromised your information
Follow the duties and privacy practices described in this notice
Not use or disclose your information other than as described here unless authorized by you in writing

We may change the terms of this Notice, and the changes will apply to all health information we maintain. The updated Notice will be posted on our website and available upon request.
